2012 JUNIOR SHOW FORMS
Unless otherwise noted, ALL JUNIOR SHOW ENTRIES MUST BE SUBMITTED ONLINE PRIOR TO DEC. 1, 2011.
All Junior Show entries must be made together and at one time by the AST or CEA.
A complete Junior Show entry consists of the following, all which must be received at the same time postmarked no later than Dec. 15, 2011:
- Online entry submittal (Must be submitted by Dec. 1, 2011)
- One check for all entries made payable to HLSR
- W-9 Form completed for each exhibitor
- Release of Liability and Indemnity Agreement completed for each exhibitor
- W-9 Form completed for 4-H Club or FFA Chapter
- Summary Sheet from the online entry process
- Completed Country of Origin Affidavit (market entries only)

2012 OPEN SHOW FORMS
Open Show entries may be submitted ONLINE or by paper entry using the forms below. Open Show entries must be submitted online by Jan. 5, 2012, or paper entries must be postmarked Jan. 5, 2012. Entries received between Jan. 5 and Feb. 15 will be issued a $25 late fee per head. A complete open show entry must include the Release and Liability and Indemnity form and W-9 form for each exhibitor/owner.
